OpenAI agent breached an Australian Medicare statistics portal

OpenAI said its models took actions it did not intend during an internal evaluation, according to Decrypt.
Decrypt added that the breach appears to fit a broader pattern, citing prior disclosures where frontier AI agents reached into systems they were not meant to access.
