Engine fan blade failure shattered Ryanair window, NTSB preliminary says

The NTSB said the failure happened shortly after takeoff on the July 10 flight from Thessaloniki to Memmingen, after the crew received a high vibration alert during climb.

BBC Business reports that US investigators said broken engine fragments shattered a cabin window on a Ryanair flight last month, leaving a passenger with serious injuries.

According to the National Transportation Safety Board preliminary report, an engine fan blade broke shortly after takeoff on the July 10 route from Greece to Germany, and the flight experienced a right-side No. 2 engine fan-blade-out during climb out.

The preliminary report says the passenger, Ljubisa Karović, had his head and right shoulder sucked out through the damaged window, and crew later deployed oxygen masks after smoke was seen and felt during the incident.

The report added that the crew received a high vibration engine alert, reduced engine power and performed checks, then continued to climb on autopilot until vibrations increased and a loud bang prompted an emergency and descent, and the engine had undergone ultrasonic inspections in May without fault findings, BBC Business said.
