Pharmacy benefits advisor churn roils 75% of US employers

A 2026 survey of 601 employers found advisor changes are driven by concerns over provider compensation arrangements and high commissions or fees.

A new survey says pharmacy benefits advisor churn is widespread among US employers, with three out of four reporting either a change in advisor over the past 12 months or plans to switch in the next year, reflecting mounting pressure around drug costs, federal disclosure rules, and employer dissatisfaction, according to Insurance Business. The findings come from the 2026 State of Pharmacy Benefits Decisions report published by RxBenefits, a pharmacy benefits optimizer based in Birmingham, Alabama. Insurance Business said the tracker was fielded by FTI Consulting from June 23 to July 21 and captured responses from 601 employers and 300 employee benefits consultants and advisors with decision-making authority over pharmacy benefits. Insurance Business reported that this is the fourth wave of the tracker, first conducted in 2023. Among the reasons employers cited for changing or planning to change advisors, concerns about compensation arrangements with providers or vendors led at 31%, tied with a lack of proactive guidance or recommendations, while high commissions or fees were cited at 30%.
